Travel Vaccinations
If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.
To help us offer the appropriate advice, please print out and complete this travel questionnaire before coming to see the nurse.
*Please note not all of our sites provide certain travel vaccines. Please ask at reception and you may be directed to book at an alternate site.
Please note that our nurses do sometimes get booked up, so please allow 4 weeks to get a convenient nurse appointment. Some courses of vaccinations take two or three months to complete so please take this into consideration when planning your journey. Vaccinations attracting a fee will only be given if paid for in advance, so please bring either cash or a cheque book with you to your appointment.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
